Formulary
A formulary is a list of drugs covered by a plan.
As a new or continuing member in our plan, you may be taking drugs that are not on our formulary. Or, you may be taking a drug that is on our formulary, but your ability to get it is limited. You should talk to your doctor to decide if you should switch to an appropriate drug that we cover or request a formulary exception so that we will cover the drug you take. For each of your drugs that is not on our formulary, or if your ability to get your drugs is limited, we will cover a temporary 30-day supply (unless you have a prescription written for fewer days) when you go to a network pharmacy. After your first 30-day supply, we will not pay for these drugs. For additional information on our transition policy, please refer to the plan's formulary.

Medication Therapy Management Program
We are pleased to offer you the Medication Therapy Management Program. This program is a service offered to you — at no additional cost — as part of your health plan. This program is designed to help you better understand your medications — increasing safety and lowering costs when possible. The Medication Therapy Management Program has two parts. One part gives you direct access to a personal pharmacist. Over the phone, he or she will conduct a Comprehensive Medication Review. The pharmacist will then work to help you better understand your medications and answer any questions you have. The second part of the program includes providing you with ongoing medication information through the mail.
This program automatically includes members that meet all of the following criteria:
Have at least 3 of the following chronic health conditions: High Blood Pressure, High Cholesterol, Heart Failure, and Diabetes
Take at least 8 chronic medications covered by your Medicare drug plan
Use medications that cost more than $3,100 for the year combined (your costs and your plan's costs).
